Knoxville Academy of Medicine Foundation chooses NinePatch™ Whole Person Care to power the Tennessee Community Resource Network

 

A Social Determinants of Health (SDOH) information technology firm was selected to streamline care coordination and service referrals through shared SDoH, Medical and Behavioral health data to improve client health for a non-profit foundation.

 

[KNOXVILLE, TN] Today, the Knoxville Academy of Medicine Foundation (KAMF), a community-focused non-profit based in Knoxville, Tennessee, announced the selection of NinePatch, the award-winning community health platform, to serve as the Whole Person Care (WPC) referrals and care coordination solution for the Tennessee Community Resource Network (TCRN).
