Multi Disciplinary Partners Provide Coordinated Social and Clinical Care in Tennessee

KNOXVILLE, TN, USA – June 29 2023. Tennessee Community Resource Network (TCRN) partners, the Knoxville Academy of Medicine Foundation (KAMF) and the East Tennessee Health Information Network (etHIN), are excited to roll out a high impact clinical and social care coordination platform, known as Tennessee Community Resource Network (TCRN) in Tennessee.

KAMF is committed to improving health equity and health outcomes through coordinated whole person care. This commitment extends to building community engagement, simplifying health and social data exchange and interoperability, and creating pathways for cross-community care coordination across medical, behavioral, mental, and social care domains. Focusing on social determinants of health fits well within their mission to support initiatives that address health equity and the improvement of health outcomes across Tennessee.

According to KAMF Executive Director, Danielle D. Sims, “metropolitan, suburban, and rural residents will receive help from clinical and community-based organizations (CBOs) working together to close gaps for needed services, make instant referrals, and coordinate care. With ongoing support from etHIN, the region’s Health Information Exchange (HIE), we will continue to advance health and well-being for all Tennesseans.”

Community-Wide Support Between Physicians and Local Organizations Will Raise Health Standards

Community clinical and social care partners, their patients, and clients will benefit from spending less time on paperwork and more time on efforts to ensure success through:

  • Use of a single shared communication tool to address social determinants of health
  • Care coordination across clinical and social organizations
  • Shared care plans that make it easy to follow up on referrals

Dr. John “Jack” Lacey III, Executive Health Outcomes Director, says, “this program represents an opportunity for community-wide support between physicians and local organizations. The partnerships that will be created have the potential to eliminate barriers and raise standards of health and daily living for all persons in the state.”
